REQUEST FOR PREQUALIFICATION OF GENERAL CONTRACTORS
For Aging in Place Program
Habitat for Humanity of Cleveland seeks to prequalify General Contractors from which to solicit bids for our Aging in Place Program. The funding for this project was provided through a grant awarded by the Davidson County Chancery Court, Part III from the SeniorTrust/ElderTrust settlement (Case No. 11-1548-III) and through a contract administered by the Tennessee Commission on Aging and Disability.
Scope includes, but is not limited to: accessibility improvements, mobility modifications, weatherization enhancements and critical home repairs. Applicable city, state, and federal regulations such as but not limited to Section 3, Segregated Facility, Section 109, and E.O. 11246 are required. The application packet consists of the following submittals, which must be submitted by 4 PM on Tuesday, November 13, 2018:
3. Certificate of Insurance with minimum $500,000 General Liability with Habitat for Humanity of Cleveland named as additional insured.
4. Certificate of Insurance for Worker’s Compensation with Habitat for Humanity of Cleveland named as additional insured.
5. Copy of Applicable Business License (General Contractor or Home Improvement License)
